Replace deprecated `uv` env vars and provide space-separated extra URLs to [`UV_INDEX`](https://docs.astral.sh/uv/configuration/environment/#uv_index) and a single default index to [`UV_DEFAULT_INDEX`](https://docs.astral.sh/uv/configuration/environment/#uv_default_index).
