Removal of logging clientName and commandName is tracked in:

• Remove logging of clientName and commandName from Command smithy-lang/smithy-typescript#249
• chore(clients): remove clientName and commandName logging from Command #1787

Description of changes:
log clientName, commandName, input, output in addition to metadata
