Helped prevent some .env usage issues

- Added extra "common issue" for failing credentials (Values that need
  to be quoted).
- Updated LDAP details to shown DN and PASS by default with quoted
  values.

Improved debugging instructions

Seperated out error log details as first header to lead users down that
path as first choice, and for easier linking during support.
Added possible webserver locations to further help debugging.
