Fix microsoft/ptvsd#78 (microsoft#960)

Fix microsoft/ptvsd#78 Fixes microsoft#794 Wait for `process` event before requesting pause. Increase timeouts when waiting for `process` and `thread`, due to pydevd delay in thread reporting.
