[rubygems/rubygems] Reduce array allocations and concatenations in Index
Remove the default nested hash in Index entirely Index#search_all now yields or returns enum since that's what caller needs.

Remove the default nested hash in Index entirely
Index#search_all now yields or returns enum since that's what caller
needs.
